Booking Policies

All appointments require your card to be held on our booking system, in case of cancellations or no shows. Your appointment will not be confirmed until I have received your card verification. Cancellation Policy

Cancellation Policy

Please provide at least 48 hours notice if you need to change or cancel an appointment, failure to comply will result in you being charged 50% off your appointment fee.

Failure to Attend

Failure to attend appointments will result in 50% of your appointment fee being charged.

Payment

Payment can be made on the day of your appointment via cash or card.

Privacy Policy

OVERVIEW

This privacy policy sets out how A-Z REHAB uses any information you provide, when informing us of your personal details prior to or during your physiotherapy or massage session.

A-Z REHAB are committed to ensuring your privacy is protected at all times. The privacy of your treatment record and data collected, will be protected and held in compliance with the General Data Protection Regulations (GDPR). A-Z REHAB may adapt this policy infrequently when updating this page. The below policy is effective from the 2nd March 2024.

WHAT WE COLLECT

As a healthcare professional service, we are legally required to collect data regarding yourself. All information provided will be processed as a medical record. We collect and transfer all data in line with good practice guidelines of the GDPR. Furthermore, we have a legal obligation to hold medical records for a period of time post treatment. Once this time frame has been completed, the records will be destroyed in line with medical data practice. All data provided is kept completely confidential and is never shared with third party providers.

Data collected can include but is not limited to:

Your name, email address, telephone number, home address, date of birth, emergency contact and medical records. All data is collected via our booking app and in person.

WHAT WE DO WITH THE INFORMATION

All information is required to better understand your condition and, help us plan treatment accordingly. Internal records are kept either electronically or via a hard copy. Information may be used to improve our service and displayed on the website with your consent.
